Programs

We deliver high-impact, grant-funded initiatives—such as the Diabetes Education and Empowerment Program (DEEP) and the Illinois Public Health Institute’s (IPHI) Integrated Community and Recreational Support (iCARES) for Diabetes Prevention Program (DPP)—to promote healthier lifestyles and reduce the risk of chronic disease. Funded by the CDC, these evidence-based programs support our commitment to sustainable, community-led health transformation.

Diabetes Empowerment Education Program (DEEP)

Diabetes Empowerment Education Program (DEEP™)

DEEP™ is a diabetes self-management education (DSME) program that has been shown to be successful in helping participants take control of their diabetes and reduce the risk of complications. DEEP™ was developed for use in low-income, racial and ethnic minority populations.

CARES Cook County

CARES Cook County: Diabetes Prevention and Management

The Cook County Collaboration to Advance Reach, Equity and Systems (CARES) to Prevent and Manage Diabetes is a multi-sector effort by the Illinois Public Health Institute to expand access to evidence-based diabetes prevention and management programs in Cook County communities that have a disproportionate burden of type 2 diabetes and lack sufficient preventive services.
