Hmhc Community Development and Outreach is a grant-funded nonprofit dedicated to diabetes care, support, and prevention. While grants help us sustain our core programs, community donations make it possible for us to expand our outreach, provide more free classes, and serve the most vulnerable communities in the Chicago Southland. Every contribution—big or small—directly supports local families living with or at risk of diabetes.
